Authorization for Medication
Palm Beach County School Board policy “prohibits school personnel from administering any prescribed medication without parental consent and a medication authorization form must be signed by the child’s physician and/or parent(s).”
In order for prescription medication to be administered to your child, we must have an Physician’s Authorization Form. This form is available at the Dr.’s office and must be kept on record at the school.
These forms must be completed by a pediatrician or a family doctor. Medication must be in its original container labeled with the following information: the child’s name, dosage, name of the drug, physician’s name, and the name and phone number of the pharmacy that filled the prescription. Rigid guidelines are followed in administering medication.
